Using the Form

The only required text field is the Name field.

The Email field will allow the system to send you an email confirmation when Revenue Cycle has processed the form.

The Date of Service and Location of Service fields will help track submissions. These will be included in the confirmation.

To upload the actual facesheet, click the Browse Files link which will give you the option for Camera. (See both images.)

Your camera will open. Capture a clear picture of the facesheet information. When you take a photo following this process, the photo will not be stored locally on your phone.

Once you have taken the photo, you have the option of checking the box to receive an email copy of your submission.
IMPORTANT: Only use your @unlv.edu email address to receive these confirmation emails to maintain HIPAA compliance.

Click Submit, and the form will reload with a confirmation that the prior one was submitted. It may take a few moments to fully submit.

Once Revenue Cycle has processed the form, you will receive a confirmation email (if you entered your email address in the form).
