Space Request Form

Instructions

  1. Use the form to request space for new staff and moves. The request form will capture staff needs:

    1. Room, cubicle, or work station assignment and furniture

    2. Keys and building access cards

    3. Parking pass guidance 

    4. Signage placard guidance

    5. Computer needs

    6. Account access needs

    7. Telecom

  2. Submit the request a minimum of two weeks prior to move. It may require additional time for multiple moves, FF&E and infrastructure needs.

  3. For multiple staff or department move requests, attach a spreadsheet to the original space request form.
