1. When an evaluation is assigned to you, you will receive a notification via email. Click the link in the email to be directed to MedHub.

2. Click the SSO (single sign-on) image to log into MedHub
with your ACE credentials.

3. If you have any outstanding tasks that need to be
completed, they will appear under Urgent Tasks in the top
left corner. Click Incomplete Evaluations.

4. Any evaluations you need to complete will be listed. Click the link of the one you want to complete.

5. Complete the evaluation form.

6. Near the Submit button is a drop down menu with two
options: You can save your responses and finish the
evaluation later, or submit your completed evaluation.

Choose which option you need and click Submit.
